Who is eligible?
Carrum Health is available to Sana PPO Plus members and dependents, ages 18 and up.
*With the exception of second opinions, per IRS guidelines, PPO Plus HSA members must first meet the IRS minimum deductible per plan year. Second opinions are provided at no cost to the member and do not require payment of any deductible. Copays and coinsurance will be waived for all services provided through Carrum. Reimbursements over $50 per day are considered taxable. You will receive a 1099-MISC form for taxable reimbursements exceeding $600 per plan year.

Carrum Health covers a wide range of non-emergency surgical and medical care from top doctors in the country who specialize in a variety of conditions. You’ll even be able to receive a second opinion from one of these experts, at no cost to you. To learn which services are available to you, log in to your account or call Carrum Health at 1-888-855-8706.

To see how much you may need to pay, complete your Carrum Health profile or connect with our care team today. Sometimes, to get the best care possible, you may need to travel to a specialized hospital or care center. Fortunately, in these instances, travel expenses like airfare, hotel fees, and even meals are often covered for you and a companion, depending on the care you need. Your Carrum Care Specialist will even make all of your travel arrangements, so all you have to do is focus on yourself. After completing your Carrum Health profile and letting us know what type of care you’re looking for, you’ll see a list of requirements for your specific case. You’ll also be asked to complete an online release form, so that necessary medical records and images can be collected by our team and sent to your doctor for review.
