It is required for Credence BCBS plan members to go through the Plan’s Carrum Health COE program for bariatric surgery procedures. Your surgery will not be covered if you get one of these surgeries outside of the Carrum Health Bariatric COE program, even if the provider is in-network with Credence BCBS.
