The following are eligible to participate in the Carrum Health Center of Excellence (COE) benefit:

Effective January 1, 2022, Carrum Health will be the preferred provider of knee/hip replacement and spinal fusion surgeries for U.S. 3M medical plan participants. Other surgeries and procedures will continue to be covered by Blue Cross and Blue Shield of MN, HealthPartners, or by Carrum Health, as applicable.

How do I participate in the program?

  1. Receive a surgical recommendation from a treating doctor for one of Carrum’s covered surgeries.
  2. Register with the Carrum Health program. Visit info.carrumhealth.com/3m, call 1-888-855-7806, or download the app on iPhone or Android devices.
  3. Select your hospital and surgeon from the list on the website or app.
  4. You will be assigned a personal care specialist to help you throughout the process.
  5. Your care specialist will help you complete the necessary forms and gather your medical records and any relevant lab work or information required for an evaluation by your surgeon.
  6. Complete a virtual consultation over the phone with your surgeon.
  7. Once you’re qualified for surgery, your care specialist will help you schedule your surgery, manage travel, if needed, and complete any necessary paperwork.
  8. After surgery, our care specialists will assist in coordinating follow-up care on your behalf.

Which services and expenses are covered?

I don’t live close to any facility. How can I participate?

Depending on where you live, to get the best possible care, you may need to travel to a specialized hospital. For procedures that don’t require you to live close to the hospital, you may be eligible to have your travel expenses covered for you and one adult companion to help care for you during travel.

Is my current doctor covered by Carrum or part of the Carrum program?

If your current doctor practices at one of the facilities covered under 3M’s Carrum Health benefit, then it is possible your doctor is a part of the Carrum Health program. To view a list of doctors, please register at info.carrumhealth.com/3m, select the procedure you need, and the list of available surgeons will be shown for your review and selection.

I already had surgery. Can I be reimbursed?

Carrum is unable to reimburse patients who have already undergone surgery. If you are currently scheduled for one of our covered procedures and are interested in switching to the Carrum program, please register at info.carrumhealth.com/3m or contact our care specialist team to ensure you meet the pre-qualification criteria. You will still need to select a hospital and be seen by a Carrum surgeon to participate and receive the financial benefits afforded by the Carrum program.

Do I need to provide medical records?

Your care specialist will collect your medical records for you. We work with a vendor that will help us gather your medical records. Once you complete the online release form, your records and images will be collected and sent to your surgeon’s medical team for review. Your care specialist will inform you about specific requirements based on your procedure to ensure your selected surgeon has the medical information needed to review your case.

What about recovery care post-discharge?

Our care specialists will assist in coordinating follow-up care on your behalf. This includes:

  • Sending your post-discharge plan and hospital records to your selected local physician.
  • Helping you select a local in-network physical therapy office if needed.
  • Sending physical therapy orders to the selected facility on your behalf.

Who is going to perform my recovery care?

In order to be eligible for the program, you must have an established relationship with a local physician. Your physician must be willing to assume ongoing care once you return home. Your care specialist will gather your home physician’s contact information and facilitate transition arrangements for all necessary follow-up care on your behalf.

Carrum Health Benefit Frequently Asked Questions

If I am on a high-deductible plan, how much do I have to pay to participate in the Carrum program?

You need to meet your deductible in full, but copays and coinsurance will be waived. If you have already paid for medical expenses throughout the year, you’ll only pay the remainder of your deductible. Our app and/or care specialists will help you determine what your out-of-pocket cost estimate will be.

Covered expenses may include airfare, hotel fees, mileage reimbursement, and a daily allowance for meals. Your Carrum Health care specialist will even make all your travel plans, so all you have to do is focus on what matters most—You. To find out if your travel expenses are covered, sign in to your Carrum Health account.

  • 3M pays for all or most of the costs for a covered surgery under the Carrum Health benefit. Those on a high-deductible plan must first meet their deductible before costs are covered. To have your surgery expenses paid, services must be authorized and scheduled by Carrum Health. Once you register on the info.carrumhealth.com/3m website, you can view a list of covered surgeries
  • Your surgical consultation, preoperative appointment, and full hospital stay from admission through discharge (or travel clearance, if traveling) are fully covered. Additional testing that may be required as part of your evaluation, such as labs or images, is not included—your care specialist will inform you if these are necessary.
  • If you need to travel to have your surgery, the Carrum Health benefit will cover expenses for you and one adult companion, including your primary mode of transportation (round trip), one hotel room (two beds), a daily allowance for meals (consistent with your employer’s travel policy), and parking.

Additional medical or financial criteria for covered procedures are also required for eligibility.
